Before you begin:

  • You cannot save this application once started. Please review the requirements first.
  • It is best to complete the application on a computer.
  • Only one application per student should be submitted.
  • You may pick up Teacher Recommendation and other required forms from your school counselor. 

What You Might Need

  • Essay (200 words): Describe why you want to join Upward Bound and how the program will help you .
  • Forms: Release of Records and two Teacher Recommendations 
  • School Records: Recent report card, transcript, test scores, or IEP (if applicable) .
  • Income Verification: Parent/guardian tax return or proof of public assistance .
  • Copy of Social Security card

Submission options:

  • You may submit documents with your application, have your school send them, or turn them in later at your interview.
  • After review, eligible students will be invited to a student + parent/guardian interview. Notification of acceptance will follow by letter or email.
  • All information is kept confidential and used only for program purposes.
